Forms and Fees
The following forms must be submitted for each project. You can download the forms here.
- OAS Consent and Release Agreement
- ISEF Form 1
- ISEF Form 1(a)
- Research Plan
- ISEF Form 1(b)

Fee - $20 per person, even if in teams. $12 goes to Ohio Academy of Science, and $8 helps cover District Day expenses. Each school must provide one judge for every three projects sent to District Day as well as one volunteer per school. Teams may have no more than three participants. Everyone must be present during the judging.
If a student does not have a local science fair, he/she may submit a complete project prepared according to the Ohio Academy of Science rules to the District 9 Science Review Committee for consideration. If the project earns a superior rating as evaluated by the judges, the project may compete in the District 9 Science Day.

For a list of rules and standards for Science Day, please see the Ohio Academy of Science website at http://ohiosci.org/sds.htm. Please note that earning a Superior at District Science Day does not automatically qualify a student for State Science Day as indicated in the rules of the Ohio Academy of Science. Please refer to the Academy’s website above for additional information.
In addition, this year District 9 is not affiliated with an ISEF event. Please refer to the Ohio Academy of Science for more information about the ISEF.
